MENTORING & INDIVIDUAL SKILL DEVELOPMENT

Peer support

Individual support grounded in lived experience, helping participants develop practical skills, confidence and independence in everyday life and their community.

Support is linked to the participant’s goals and may include:

  • Mentoring through life changes and transitions.
  • Building skills to plan daily activities and manage appointments.
  • Developing confidence in communication, decision-making and self-advocacy.
  • Strengthening social connections and support networks.
  • Planning steps towards community participation and greater independence.

For plan managers and support coordinators: This service focuses on mentoring, peer support and individual skill development. We discuss the proposed activities and relevant funding arrangements before agreeing to services.

RECOVERY PLANNING & COORDINATING SUPPORTS

Psychosocial recovery coaching

Recovery-focused support for participants with psychosocial disability, helping people take greater control of their lives and navigate the supports involved in their recovery.

Working alongside the participant and, with their consent, people important to them, recovery coaching may include:

  • Developing a recovery plan with meaningful goals and practical steps.
  • Building strengths, resilience and confidence in making decisions.
  • Understanding the NDIS plan and organising relevant supports.
  • Connecting with community, health and mental health services.
  • Helping services work together and maintaining engagement when support needs change.

For plan managers and support coordinators: This is a separate service from peer support, delivered where appropriate psychosocial recovery coaching funding is available. The scope of work is agreed before services begin.
